INSTRUMENTS OF INVESTIGATION
AIMING TO GIVE YOU THE IDEA OF HOW WE WORK WITH OUR PATIENTS, PLEASE FIND HERE A SERIES OF INSTRUMENTS AND PASSAGES WE ADOPT IN OUR PRACTICE.
One of the first steps we take together with our patients consists in understanding what their problem is, and especially what brought about their actual discomfort or pain. The investigation we carry out is very specific: what we do is reconstructing “the history” of the patients in order to find out the causes, the concauses, the predisposing factors and the triggering factors of their condition. We follow this working procedure since each effect is always preceded and produced by a main original cause and goes together with specific predisposing factors related to that cause. Apart from the history of the patients, our posturologists need to be able to listen to the verbal language of the patients and to observe their body language. No gesture, or inflexion, is used by chance. Men’s intelligence is proportionate to their ability to “observe and solve problems”, and not to the idea of “knowing everything”. After completing this phase, we proceed with the postural evaluation.
POSTURAL INVESTIGATION AND EVALUATION
At this point, we observe how the patients move, stand, stand up, and sit down. We concentrate on how they relate to the therapist, on how they move their feet and their arms. Then we observe the curves of their column, the position of their head, neck, and shoulders. We evaluate the balance of their pelvis, any eventual valgus or varus condition, or genu recurvatum, then the condition of their halluces and of the arches of their feet, etc.
Each “body wrinkle” has its own meaning…
The next step involves the tests.
INVESTIGATION THROUGH THE TESTS
Different tests are carried out to check the mobility of neck, shoulders, torso, hips, knee, and ankles. For any joint resulting stiff or blocked there is an explanation, and any block will involve in turn other districts, dragging the whole system towards a pathologic condition. To solve the problems present in some parts of the body will allow to solve or even to prevent problems concerning the whole system.
The next step is to test eventual scars, the swallowing, the visual function, occlusion, and the function of the temporomandibular joint. Then we check the position of the coccyx, since it is strictly connected with the dura mater and therefore with the meninges, with the curves of the column, and with the plantar support. Knees are tested an evaluated as well, because their condition has a remarkable influence on the shape and on the position of the feet. A test we use is the Unterberger test, whose aim is to evaluate the vestibular system by identifying the tensions of the kinetic chains while the patients are asked to walk in place keeping their eyes closed. It is useful to evaluate the functionality of the crossed nervous system. The interference of the occlusive and temporomandibular relation is also tested.
EVALUATION THROUGH THE PODOSCOPE
This kind of evaluation, apparently very simple but relevant, is carried out when it is useful to have a confirmation of an altered foot placement or of a foot pathology. It also allows comparing the situation of the foot before and after the postural treatments. It has to be said that over 90% of foot problems has a descending origin.
PHOTOGRAPHIC EVALUATION WITH BARRÉ TEST OR SCOLIOSOMETER
In order not to have doubts on eventual asymmetries or morphological alterations, photographs of the plumb-line or laser are taken. Photographs taken on different planes will help to get to the next considerations.
STABILOMETRIC AND BAROPODOMETRIC PLATFORM
Whereas clinics has the ability to observe the phenomena, it is with instruments that it is possible to show and prove the condition of an imbalance.
These electronic tests allow to obtain a “photograph” of the present situation in a repeatable way and to observe eventual changes in case specific techniques or maneuvers are carried out. It means that the data we get from these tests are objective and cannot be influenced by subjective factors. Therefore we can evaluate the distribution of the load on the ground and the answers upwards, finding indications or suggestions in order to act in the best possible way and in less time.
KNEE ROTATION MEASURER (KRM), A MEDICAL DEVICE FOR MEASURING LEGS/KNEES/FEET
This innovative medical measuring device provides useful information to evaluate the condition of the inferior limb. It allows us to evaluate in a very precise way the level of a valgus and varus condition, genu recurvatum, genu flexum, internal and external rotations of the knee. With this device it is very simple to show all the improvements deriving from postural treatments and therefore also to appreciate the final results obtained.
CERVICAL TEST AND ARTICULAR RANGE OF MOVEMENT (ROM)
With this electronic device it is possible to measure precisely and quickly the mobility of the cervical/cranial/back/lumbar tracts, both while flexing, rotating, and inclining. The same can be done with any other joint. Also in this case, the improvements can be kept under control.
These are tests used to evaluate the neuromuscular answer to a physical stimulus.
Although such tests are thought to be too simple and not scientific, they are actually used all over the world because of their immediacy and their immediate answers, two elements that deserve to be taken into consideration. Kinesiology tests are also known as tests of muscular power and their results allow to identify the irritative thorns and to act on them, thus accelerating the process of postural rehabilitation.
If the swallowing is incorrect, the tongue and the tongue muscular chain are likely to cause remarkable disturbance to all the neuromuscular system. In order to evaluate adequately the swallowing and to find out whether it is correct or there is something wrong in it, it is useful to use some tests. With the help of these tests, therapists can create a cycle of both structural and functional treatments aiming to improve patients’ health and life quality. We have two experts who deal with postural rebalance related to swallowing re-education. Given the important relation between tongue and teeth, very often patients are visited also by a competent doctor.
Depending on the physiology of our mouth, cranium, cervical tract, temporomandibular joint, we could have correct or incorrect functions of the mouth. The teeth need to have the right spatial organization, in relation with the arches and the tongue, and to be well-balanced with superior and inferior arch. These tests, which can result complex sometimes, allow to understand whether the occlusal relations are incorrect and therefore if there are repercussions on the whole postural system. Tests also tell us whether the condition of the mouth is the cause or the effect of postural problems.
BASIC VISUAL TESTS
Using optometric and postural indications, basic tests of the visual function are carried out in order to evaluate whether the extrinsic muscles of the eyes are able to fulfill their task or if they are disturbed. Through these tests it is possible to understand whether the eyes are the cause of postural problems or victims of those and by doing this it becomes evident when a visit to the optometrist could be useful.
QUANTUM PHYSICS TESTS Thanks to some bioelectronics tests carried out with specific equipment it is possible to evaluate the energetic status of organs and even of energetic meridians. These tests allow evaluating every single organ and apparatus in order to understand their efficiency and their influence on posture.
